As mentioned before, there are three components of facial aging and the loss of volume in the face. Fat loss, muscle loss, and bone loss play a major role. It seems that part of the volume is lost, but part of it gets displaced from one area to another. The facelift operation should deal with restoring some of the volume displacement, but there is definitely volume loss that needs to be dealt with as well. Luckily, today we have many fillers to choose from, each offering a different characteristic so that we can once again decide which one works best for a certain area of the face. Filler injection is practiced by all kinds of physicians, but the results vary greatly depending on the training dedication and artistic sense of the injector. Dr. Rafizadeh, with his long experience as a plastic surgeon and as an artist is best positioned to give you the best possible results. He would do this the safest and the most effective way with minimal to no bruising and pain. Once you experience filler injections with Dr Rafizadeh, you would never have it done elsewhere.
Before this great product came on the market, there was bovine collagen and then human collagen. None of these fillers worked well in that they did not last long enough. Thus, Dr. Rafizadeh stopped using them altogether because he was not satisfied with the results. He was using fat grafting in the lips instead. Restylane, a hyaluronic acid, truly revolutionized the filler industry. Here was a product that you could depend on to last at least six months and in some patients a lot longer. This being said, it is very patient-dependent. In fact, it seems that some patient do not break down the product for a long time. Today, Dr. Rafizadeh uses Restylane in specific areas: mostly the lips, superficial wrinkles, and the tear through area.
This product, like Restylane, is a hyaluronic acid (HA). But, it has more cross links to last longer. This is a good volumizer in the nasolabial folds and the cheeks.
Juvederm Ultra and Ultra Plus
These are another HA product. Juvederm Ultra Plus can be a long-lasting filler which lasts over a year in some patients.
This is an excellent volumizer for many areas of the face, except the areas with thin skin such as the lips and around the eyelids. Radiesse is made of calcium hydroxyapatote, a bone constituent that is in suspension in a gel. It immediately shows results, but over time, as the gel dissolves, there is collagen production replacing it to maintain the volume. Dr. Rafizadeh uses this product routinely for the enhancement of the cheeks, the nasolabial folds, the area in front of the jowls, and for chin augmentation etc. All the HAs come with a local anesthetic included in the syringe, but with Radiesse we add a local anesthetic and mix it with Radiesse to reduce the pain of injection. Several years ago, Dr. Rafizadeh started doing all the deep filler injections with a blunt cannula. This cannula allows for a painless process and causes minimal bruising. This is also a safer method because it avoids an intra-vascular injection.
Juvederm Voluma XC
Voluma the newest filler, volumizer from Allergan is a welcome addition to the number of good products available. This is primarily designed for cheeks and allows the creation of a defined and raised cheek bones. The injection is to be done deeply near the bone or deep subcutaneous tissues. Dr. Rafizadeh uses the micro-cannula to increase safety and avoid pain and bruising. If this product lives up to what it claims to be It would be the most important tool in creating volume in the face. We hope that this product will last the two years that the manufacturer is suggesting. WQe are so far very excited to have Voluma available to our patients.
